Packet hygiene: send it complete, once
Most avoidable denials trace back to a packet that was missing one item. A fixed routine beats memory.
- Start from a checklist. Use the Auth Packet builder for the request type.
- Match the order to the payer form. Reviewers read top to bottom; put the order and diagnosis first.
- Name the evidence. If the payer asks for failed conservative therapy, state the type, length and outcome in the notes.
- Keep proof. Record the date, method and confirmation or reference number of every submission.
- Do a second-person check before sending.
Worked example. An imaging request is returned with "clinical information incomplete". The packet had a diagnosis code but no notes. Fix: add the note describing symptoms and six weeks of physical therapy, then resubmit and record the new reference.
